How can a Cancer Care Coordinator help?
Your Cancer Care Coordinator will contact you in the first 3 months of your diagnosis to introduce their role in your cancer journey. This is your opportunity to ask any initial questions or raise any concerns you have in the early stages of diagnosis.
Your Cancer Care Coordinator will contact you again within the first 6-9 months after you are first diagnosed, usually after your hospital treatment is complete. They will invite you to a Cancer Care Review. This is an appointment where you can freely discuss the questions or concerns you have that are related to your cancer diagnosis. This appointment is led by you and is a safe space to express how you are feeling.
What is a Cancer Care Review?
A Cancer Care Review (CCR) is a conversation between a patient and their GP or Practice Nurse about their cancer journey. It is essential to personalised care and helps patients to talk about their cancer experience and concerns, and understand what support is available in their community.
Cancer Screening
The Cancer Care Coordinators also help the GP practices to encourage people to take up cancer screening.
Cancer screening means that cancers can be caught early. The earlier you are diagnosed, the earlier you can get treated and the greater chance you have of beating that cancer.
If you are worried or have questions about screening then ring your GP surgery and ask to speak to either a cancer care coordinator or one of the practice nurses.
Everyone with a cancer diagnosis is entitled to a Cancer Care Review and further support from their GP Practice. If there is no cancer care coordinator at your surgery, your GP or other member of the practice team can see you to discuss any concerns you have about your cancer diagnosis.
