What is social prescribing?
Social prescribing is a way of helping people to be given the time to focusing on ‘what matters to them’ and taking a holistic approach to look at someones health and wellbeing as a whole. It also connects people to community groups and other services used through a link worker or wellbeing worker. It is all about working collaboratively to get people the best practical and emotional support.
This can work for a wide range of people, including people:
- with one or more long-term conditions
- who need support with their mental health
- who are lonely or isolated
- who have complex social needs which affect their wellbeing.
Social prescribing can reduce the need to access NHS services such as the GP and A&E as it connects people to support in their area to improve their overall wellbeing. Join us and find out about the support and training available.
