Nottingham and Nottinghamshire ICB’s Personalised Care Programme is a success!

This two-year programme aimed to reduce surgery cancellations and improve patient outcomes.

Here’s what we achieved:

  • Improved patient health and well-being: Patients received vital support, including mental health services and financial assistance through grants.
  • Reduced cancellations: Our Link Workers helped patients overcome barriers to surgery, leading to more procedures happening on schedule.
  • Faster recovery: Patients felt they could return to work and daily life sooner after surgery.
  • Motivated staff: The programme created a positive atmosphere for healthcare professionals.
